Lotus Train: Dong Hoi → Ha Noi
The late northbound departure, built around a full day in Phong Nha. Boarding is close to midnight, so you can finish the afternoon in the caves, have dinner in Dong Hoi and still make the train without cutting anything short. You arrive in Ha Noi before midday, which is the useful part: enough time to check into a hotel in the Old Quarter, or to make an evening flight out of Noi Bai without spending the day in a departure lounge.
